OUT signal highly unstable

Using this to control the position of a linear servo. Reading the OUT signal via ADC and using that to modulate servo position. All connections made as stated. Vcc = 5V from arduino. Confirmed with oscilloscope that the 5V supply was stable. OUT signal is highly unstable (measured with scope directly from the Grove pcb terminator). DC swings of 1V and random spike artifacts, even while the slider is stationary. I only purchased one of these, so it could be that just this particular unit is defective.
